POLICY: Reporting Injuries

PURPOSE: To establish a uniform procedure for reporting injuries.

RESPONSIBILITY: Superintendent


If an injury occurs:

1.Obtain appropriate medical treatment.

2.Complete the “First Report of Injury” form (Form O.F.M.-250-A).

3. Call Rodney Herald with McGriff, Seibels & Williams at 205/581-9115 within twenty-four (24) hours and report the injury using the information you listed on the above mentioned worksheet. Immediately after calling and reporting the injury, fax the “First Report of Injury” to Rodney Herald at 205/581-9195.

4. Contact the corporate office and report the injury to the Director of Field Operations, the Senior Project Manager, or the President.

5. Mail the worksheet to the corporate office to the President’s attention with a copy to the Safety Department and the Accounting Department.
